Leaseholder Information
Please notify HG Energy, LLC in writing if you need to make the following changes:
- Address Change
- Lease Division Orders
- Tax ID Number or Social Security Number Change
- Transfer of Ownership
- W-9 Form

If obtaining information on behalf of the Leaseholder or Royalty Owner an Authorization to Release Personal Information form must be completed.
